Transformation Plan of Care (TPOC)

Building on what we have learned and achieved through our LEAN and Flo initiatives, HDGH is embarking on an enterprise wide Transformational Plan of Care which will allow our organization to build capacity through enterprise wide improvements. It will create our blueprint for moving forward and creating a high performing hospital of which can all be proud.

Over the past three years we have had several successful LEAN projects that have improved our processes and the way we do the work. These LEAN exercises however, for the most part, have occurred in isolation. What has been missing to this point is an ability to tie this work together and make enterprise wide connections and system wide sustainable improvements.

A TPOC is a transformational guide that uses A3 (LEAN) thinking—a problem solving method. It is a tool that assists an organization to create an irreversible culture change. It helps an organization to align all of its elements in a set direction. It is a tool that helps to focus its management team on targeting the right improvements. A TPOC defines an organization’s current state, its ideal target (future) state and at a high level the path the hospital will need to take to achieve that target.

It helps us to define the gaps between our current and target states and how we will go about addressing them. TPOC also assists us to identify the underlying measures we will use to monitor the improvements. During the TPOC process, several value streams that will help us have the biggest impact are identified and metrics are established to assist the management team to evaluate them.

The analysis of the value streams identify the main focus areas for Rapid Improvement Events (RIE) that will be conducted at a rate of two or three each month for the year. RIEs are the main vehicle for experimentation -- helping staff to develop new thinking and practicing skills. Participants in these events will be drawn widely from among interested staff in all roles of the hospital.
