Patient Safety

At Hôtel-Dieu Grace Hospital patient safety is a key strategic priority. The paramount importance of patient safety is reflected in our vision and strategic plan and it is also embedded into the job descriptions of everyone employed by the hospital. Our board of directiors has established a Patient Safety and Community Care Committee that comprises, physicians, nurses, administrators, board members as well as members of the community at large. This committee meets monthly and reviews patient safety related indicators and issues.

Each quarter, the hospital in conjunction with its Patient Safety and Community Care Committee publishes the Patient Safety Monitor.
This report is released publicly at public hospital board meetings and is posted on the HDGH internet. It presents an overview of the hospital's performance on key patient safety indicators including:

    Medication Reconciliation
    Surgical Site Infection
    Central Line Infections
    Critical Care Outreach Team
    C-diff infection rates
    MRSA infection rates
    VRE infection rates
    Falls
    # of Medication Incidents
    Postoperative Pulmonary Emboli or Deep Vein Thrombosis
