What is a Care Partner?
A Care Partner is a family member, relative, or close friend who acts as a primary support to you when you are in hospital.

At Hôtel-Dieu Grace Hospital, our goal is to provide compassionate, patient-centred, holistic care through our interdisciplinary team of healthcare professionals. We understand that for many, admittance to hospital can create feelings of uncertainty and anxiety at a time when they may be in pain or feeling poorly.

A Care Partner can provide a second pair of ears and helping hands to help your healthcare team provide more personal care for you, and they can help communicate important information between you, your family, and your healthcare team. Furthermore, your Care Partner can provide comforting emotional support and help assist with your transition back home.

Your Care Partner will have special identification that will allow them to be with you from 8am to 8pm, or possibly longer.

Care Partner Rights and Responsibilities
Your Care Partner will have a variety of rights and responsibilities:

RIGHTS:

They will have the right to extended access to you

They will have the right to assist with your care

They may access information regarding the status of your care

RESPONSIBILITIES:

They will act as the primary contact between you, your family, and your healthcare team

They will assist with planning your care and discharge home

They will help ensure that family members and friends understand the visitor policy and other hospital regulations

How To Appoint A Care Partner
Either during the pre-admission planning for your stay or shortly after an unplanned admission to HDGH, a hospital representative will ask you if you would like someone to act as your Care Partner. If you are unable to communicate with the representative, we will ask your family to recommend someone to be designated as your Care Partner.

Consent is required from you or your legal guardian to allow your loved one to act as your Care Partner. They will receive special orientation and identification from your nurse.
