Social Work

Social Workers assist patients and families during a hospital stay by supporting the patient's overall emotional and social well-being. They are professionally trained at the Baccalaureate or Masters level in individual, marital, family and group counseling, and are members of the College of Social Workers and Social Service Workers.

As members of multidisciplinary teams, Social Workers provide services in many areas of the hospital through:

Counseling

Advocacy

Initiating Community Referrals

Initiating Placement

Patients with anxiety related to illness or a traumatic experience, caregiver stress, parenting issues, resource information and difficulty coping might ask to see a Social Worker.

Resource Utilization
Our Resource Utilization team ensures that patients who require acute care access the appropriate beds. To achieve this they may:

Liaise with community partners to ensure "continuity of care" for patients.

Repatriate patients back to their city, province, and land of origin if the patient is not a resident of Windsor/Essex.

Support discharge-planning on/or before admission to hospital.

Review long-stay patients.

Identify any “medical barriers" to discharge and problem-solve to work around these barriers.
