Clinical Pathways are interdisciplinary Plans of Care that outline the sequencing and timing of therapeutic interventions and expected patient outcomes, derived from available evidence and current best practices. They are comprehensive, integrated clinical process tools, employed to optimize patient care and to help ensure positive outcomes. Our implementation process requires that they be continuously evaluated and improved.
The majority of clinical pathways have corresponding Preprinted Physician Orders. As well, each pathway has a Patient Pathway or Patient Information pamphlet that is available for patient/family education.
