Hospitalist Program

The Hospitalist Program was created to serve unattached patients who either have no family doctor, or whose family doctor does not have "active" hospital privileges at HDGH. The program was developed in response to a gradual decline in the number of Family Physicians willing to travel to hospital to visit patients or assume responsibility for "unattached" (patients with no Family Doctor) or "orphaned" patients.

We established two hospital based programs to provide care for patients while they are in hospital who have been admitted without a Family Physician: one program serves Medical/Surgical patients, and the second provides care to Mental Health patients. Celebrating 10 years of service in September 2009, the Hospitalist Program at HDGH is one of the longest running programs of its kind in Ontario.

Program Goals
  1. Support a patient and family focused model of care, while optimizing the quality of care.
  2. Ensure comprehensive service from admission through to discharge with a multidisciplinary approach.
  3. Promote the efficient utilization of resources.
  4. Coordinate services with other stakeholders in the community.
  5. Improve the satisfaction of all stakeholders: patients, families, physicians, nurses, and community partners.
