This position inovles completeing the intake process for children and youth who are experiencing emotional/behavioural/social difficulties and their families. Through active liaising with parents (and/or collaterals) and community services, the Mental Health Social Worker I works with a multidisciplinary team to complete the initial intake screening to determine service needs.
In addition, the Mental Health Social Worker I will provide professional and confidential services through the Urgent Response Service in a comprehensive, flexible, and timely maner, including: screening and intake of children/ youth with Autism who request the service; providing service navigation for all children/ youth referred; and leading the development, monitoring and evaluation of the Urgent Reponse Service Plan for those who meet the criteria for the service.
The Mental Health Social Worker I will use a family centered approach; with a focus on each individual’s strengths; demonstrate Hotel-Dieu Grace Healthcare’s stated values and service principles; and the support cross-sectorial connections and planning around the unique needs of each child, youth and family.
Co-facilitation of treatment groups for children/youth may be requested.
Determine eligibility of referrals for children/youth diagnosed with autism who request the Urgent Response service and complete the URS screening and intake process
Engage parents/caregivers and child/youth in the process of gathering intake information; build on information others have gathered to reduce the family having to repeat their story.
Provide direct services to children, youth and families, including consultation and system navigation support, as required.
Lead the development, monitoring, and evaluation of the Urgent Response Service Plan for those who meet criteria
To participate in regularly scheduled supervision and seek out supervision and consultation as needed to provide effective services to assigned children/youth and families.
Responsible for the completion of relevant reports in accordance with established standards (e.g., progress notes, assessment reports, etc.)
To promote constructive relationships amongst service providers and children, adolescents and families
Attendance at scheduled Departmental and Service/Program meetings.
Participate in staff in-service training and consultation with other staff.
SKILLS & ABILITIES:
Experience working with children/youth with multiple and/or complex special needs including autism.
Knowledge of child development.
Strong interpersonal and communication skills.
Commitment to strength-based principles of child, youth, and family centeredness.
Experience working with the child and youth service system.
Demonstrated ability to build and maintain relationships with diverse stakeholders.
Ability to work independently and contribute to a team.
Ability to maintain strong professional boudaries.
Strong organizational, verbal, written and reporting skills.
Experience working with individuals experiencing trauma.
Must have a demonstrated ability to make critical decisions quickly.
Education and/or Certification:
Bachelor’s Degree in Social Work (BSW) with clinical skills in the areas of children/youth and families and minimum three years’ work OR equivalent experience with vulnerable or at-risk populations including special needs
Current Certification of Registration issued through the Ontario College of Social Workers & Social Service Workers.
