Hôtel-Dieu Grace Healthcare is a 287 bed regional provider of all post-acute services in Windsor-Essex. HDGH offers services in Mental Health & Addictions; Rehabilitative Care; Complex Medical and Palliative Care; and Children and Youth Mental Health. We offer a unique blend of services including, but not limited to, community and home-based services. In collaboration with our healthcare and inter-sectorial partners, Hôtel-Dieu Grace Healthcare is providing care in new ways and in new locations throughout the region to address barriers, improve access and patient outcomes and improve the overall patient experience.
The Mental Health and Addictions Urgent Care Centre (MHAUCC) is intended for individuals aged 16 years or older experiencing a mental health or addiction crisis or who are having an acute and serious need, who are at risk of worsening mental health conditions that may require future hospitalization. Services include, Brief Psychotherapy and Psychosocial Support, Urgent Psychiatric Assessment and Consultation, Medical Assessment and Monitoring, Connection to Community-Based Services, Addiction Management Services.
The MHAUCC provides comprehensive team based care. This position will join our interdisciplinary team providing specialized short-term crisis intervention, risk assessment, rapid stabilization and symptom reduction.
In addition to the MHAUCC, there are various outpatient services that are also provided to support the program. These include Mobile Crisis Response Teams, Addictions Assessment and Referral, Wellness Program and Withdrawal Management.
• Providing support in the provision of physical health and psychiatric services within the interdisciplinary teams
• Assessing client condition and needs based on interviews, interactions and direct observation and physical examination, contributing to the development and implementation of therapeutic client care and transitions plans
• As an essential member of the inter-professional team, fulfilling his/her role in the evolving evidence-based health team approach to accessible, safe and effective client care.
• Developing therapeutic rapport with clients, families and community partners
• Contributing to client assessments e.g. gathering collateral data, taking histories
• Documenting client care and compliance with quality standards (under physician supervision)
Education and/or Certification:
· Graduate of a Physician Assistant Program (Canadian/US) or equivalent.
· Certified by the Physician Assistant Certification Council of Canada
· Eligible to work in Canada/Ontario
· Demonstrates ability to communicate effectively and with respect in an oral and written format with patients, public, colleagues and other health disciplines
· Demonstrates ability to perform complete and appropriate clinical assessment and development of a treatment plan in accordance with medical directives
· Demonstrates proficient and appropriate use of diagnostic and procedural skills
· Demonstrates effective problem solving and critical thinking skills
· Demonstrate ability to work in a collaborative care model with all members of the health care team
· Demonstrates commitment to continuing education activities supporting ongoing professional learning
