Residential Rehabilitation and Treatment Program

If you are experiencing a mental health emergency please contact Community Crisis Services at (519) 973-4435 or go to your nearest Emergency Department.

The program serves individuals ages 16 years and older with serious and persistent mental illness, discharged from a specialized mental health inpatient unit with a history of repeated failed attempts at more independent or group living arrangements in the community. The goal is to transition the individual from extensive hospitalization to community living environment.

The program has an eight bed capacity with a maximum length of stay of two years, providing individualized, structured programming that empowers and equips individuals with daily life skills that facilitate their return to the community; intensive skill development and support to increase their level of independence. Supportive care is available 24 hours and provided in a group-living residential environment.

Services are provided by a multidisciplinary team including an Occupational Therapist, Registered Nurses, Registered Practical Nurses, Attendants, Recreation Therapist, and Psychiatrists.

Referrals are accepted from long-term mental health facilities.
