Pulmonary Rehab
The Pulmonary Rehabilitation program is a 16 week long program that assists people diagnosed with moderate to severe Chronic Obstructive Pulmonary Disease (COPD) and other respiratory conditions. The program consists of educational sessions, taking place three times per week. The goal of the program is to assist individuals to maintain or improve quality of life and exercise tolerance, reduce dyspnea and hospital readmission rates and avoid Emergency Department visits. This program is also an essential component for individuals waiting for lung transplant.
The program is operated by an inter-professional team comprised of disciplines including physiotherapy, respiratory therapy, pharmacy, dietary, clinical ethics, speech language pathology and occupational therapy. Referrals are only accepted from a respirologist (lung specialist).
Referrals
A referral form is necessary to join the program, as well as pertinent medical documentation. Completed referral forms and medical documentation can be faxed to (519) 257-5277 for processing.
Central Registration
Upon arrival to HDGH please present to the Central Registration Department, prior to proceeding to your appointment. Central Registration is located within the main entrance, across from the Courtyard Café.
Please arrive 15 min before your appointment in order to allow enough time to register.
