Pulmonary Rehab
The Pulmonary Rehabilitation program is a 16 week long program that assists people diagnosed with moderate to severe Chronic Obstructive Pulmonary Disease (COPD) and other respiratory conditions. The program consists of educational sessions, taking place three times per week. The goal of the program is to assist individuals to maintain or improve quality of life and exercise tolerance, reduce dyspnea and hospital readmission rates and avoid Emergency Department visits. This program is also an essential component for individuals waiting for lung transplant.
The program is operated by an inter-professional team comprised of disciplines including respiratory therapy, pharmacy, dietary, clinical ethics, speech language pathology and occupational therapy.
Referrals
A referral form is necessary to join the program, as well as pertinent medical documentation. Completed referral forms and medical documentation can be faxed to (519) 257-5144 for processing.

Registration
Upon arrival to HDGH please present to the Registration Department (formerly Central Registration), prior to proceeding to your appointment. Registration is located directly to your right when entering the main lobbyo f the Dr. Fouad Tayfour Regional Rehabilitation Centre (Tayfour Building).
Please arrive 15 minutes before your appointment in order to allow enough time to register.
