Coordinated Service Planning (CSP)

CSP is available to assist children or
youth and their families with the coordination of services within Windsor Essex.
This is a voluntary service for children or youth with multiple and/or complex
special needs, who are under the age of 18, or between the ages of 18 and 21
and remain in school. The CSP team will work with families to determine their
strengths and needs after receiving referrals from community partners. Families
of CSP will be connected with Coordinated Service Planners who will connect
families with the right services within Windsor Essex and monitor how children
and youth are working through their service plan.
- To assist families with not telling their story to multiple
providers, provide navigation assistance and to give families a clear point of
contact for their child’s plan of care.
- To create a single Coordinated Service Plan that is directed by the
child/youth and family to ensure all providers are working towards the same
Who does this program serve?
CSP is best suited for children or youth with multiple and/or
complex special needs, under the age of 18, and young people between the ages
of 18 and 21 who remain in school. A diagnosis is not required to access
Coordinated Service Planning.
Referrals to Coordinated Service Planning can be made at any point a
child/youth’s needs are recognized to be multiple and/or complex. This could
- early on, when developmental
concerns are first identified, or
- When the family’s situation
changes, when new needs are identified, or when new services are added.
- Agencies throughout Windsor Essex can refer to the CSP at HDGH.
Further, if a parent or caregiver believes they could benefit from the program,
they can reach into the CSP coordinators at 519-257-KIDS (5437).
- Referral Package
This program is funded by the Ministry of Children, Community, and
Social Services. There is no cost to you for this service.
